Instructions for Completing Forms On-Line
Use one of the following options:

Most forms can be Downloaded.
Either right click to save file to your hard drive for later use or
click to open.

Most forms can be Printed.
Print, enter all requested information and sign.

| 3. |
Some forms can be Completed on-line.
Click where you want to start and begin typing.
Once a field is complete, tab to the next available field and enter the
information, or use the mouse to click on any field.
Finally, print and sign. |
Then deliver (by mail or person) the completed and signed form to the
appropriate office or department.
You will need Acrobat Reader for most forms
If you don't already have Acrobat Reader, click on the Adobe Acrobat Reader
icon and follow the instructions for downloading it from their site. This will
give you free use of Acrobat Reader 4.0.
